Privacy and HIPAA notice

How Part of Hope Behavioural Health handles your health information, what rights you have over it, and what this website does and does not collect.

Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our duties

Part of Hope Behavioural Health is required by law to protect the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We are also required to notify you if a breach affects your unsecured health information.

How your information may be used and disclosed

For treatment

We use your health information to provide and coordinate your care. For example, your provider may review your history and current medication in order to make a prescribing decision, or may share relevant information with another clinician who is treating you, with your knowledge.

For payment

We may use and disclose your information to bill and collect payment for the care you receive, including confirming coverage and providing information needed for a claim.

For health care operations

We may use your information to run the practice safely and lawfully, including quality review, training, and administrative functions.

Where the law requires or permits disclosure

There are limited circumstances in which we may be required or permitted to disclose information without your written authorization, including:

  • Where there is a serious and imminent threat to your safety or the safety of another identifiable person
  • Where we are required to report suspected abuse, neglect or exploitation of a child or vulnerable adult
  • In response to a valid court order, subpoena or other lawful process
  • For public health activities, health oversight and certain law enforcement purposes as permitted by law

Outside of these situations, most other uses and disclosures require your written authorization, and you may revoke that authorization in writing at any time. Psychotherapy notes, where they are kept, receive additional protection and generally require your specific authorization before they are disclosed.

Your rights over your information

  • Access. You can ask to inspect and receive a copy of your record. We may charge a reasonable, cost-based fee for copying.
  • Amendment. You can ask us to correct information you believe is inaccurate or incomplete.
  • Accounting of disclosures. You can ask for a list of certain disclosures we have made.
  • Restriction. You can ask us to restrict how your information is used or shared. We are not required to agree to every request, but we will consider it.
  • Confidential communication. You can ask us to contact you in a particular way, for example only at a certain number, or not to leave voicemail.
  • Paper copy. You can ask for a paper copy of this notice at any time.
  • Complaint. You can complain to the practice, or to the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalised or treated differently for complaining.

Care for clients under 18

Where a client is under 18, a parent or guardian is normally involved in arranging and consenting to care, and generally has rights over that young person’s health information. There are situations in law where a young person’s information is treated as confidential to them. Where that applies, it will be explained at the time.

This website

What the site collects

This website is a set of static pages. It does not run advertising trackers, it does not build a profile of you, and it does not ask you to create an account. The appointment request form on the contact page is not submitted to any server or third-party form service. It opens your own email application with the details filled in, so nothing leaves your device until you choose to send it.

The site loads web fonts from Google Fonts, which means your browser makes a request to that service in order to display the page. Standard server logs kept by our hosting provider may record your IP address and browser type for security and reliability purposes.

Email and web forms are not secure

Ordinary email and website forms are not secure channels for health information. The practice email address is a general mailbox, not an encrypted patient portal. Please send only enough to arrange a call, and keep clinical detail for your appointment. If you email us, you are choosing to accept that risk for the content you send.

Where this site links to another organisation, that organisation has its own privacy practices and we are not responsible for them.

Emergencies

Part of Hope Behavioural Health does not provide emergency or after-hours crisis services, and email is not monitored around the clock. If you are in crisis, call 911, or call or text 988 for the Suicide & Crisis Lifeline.

Changes to this notice

We may change this notice, and any change will apply to information we already hold as well as to information we receive in future. The current version is always available on this page, and a copy can be requested from the practice.
