This is the PHQ-9, the Patient Health Questionnaire. It is one of the most widely used depression questionnaires in the world, and it is the same one most clinicians will use with you.

Your answers stay on this page. Nothing is sent to us, saved, or stored anywhere. Closing the tab clears it. If you want us to see your result, take a screenshot or write the number down and mention it when you book.

This is a screening tool, not a diagnosis. It cannot tell you whether you have a condition. Only a full assessment with a qualified clinician can do that, and several other things produce similar answers. Please treat the result as a prompt for a conversation.
