This is the GAD-7, the Generalised Anxiety Disorder questionnaire. It is the standard short measure for anxiety and the same one most clinicians will use with you.

This is a screening tool, not a diagnosis. It cannot tell you whether you have a condition. Only a full assessment with a qualified clinician can do that, and several other things produce similar answers. Please treat the result as a prompt for a conversation.
